Planned to be released with a front cover/inlay and yellow tape labels. The red and black cover picture taken from ebay is the "Demo Split" tape listed below, and the cover/inlay distributed on the internet with a band photo on the cover is a bootleg, that the band had never seen before it surfaced on the web in recent years. The actual cover art and yellow labelled tapes were only produced in one small run of about 80, before the masters and most copies were lost in a fire. The band proceeded to remake the demo with no covers and blue labels, dubbed from an original to the new tapes on a boom box. Hence, most copies (blue label) have very low levels and loud hiss. The original yellow-label ones with art however have the original levels and the recoding is in its fully-produced original form. These copies also start with the 24-track version of "Pain & Suffering", although it isnt printed on the labels.

Recorded on a 24-tracks at Riverside Studio in Cleveland, Ohio in February 1990. After its completion, there was a fire at the studio, and all the reels were destroyed. One of the few surviving original copies was used by the band to be duplicated onto a second run of tapes (sometimes on boom boxes and at home in other means) and still sold until November when "Terror Entombed In Terror" was finished and released the same way.
